Parents/Guardians,
 
Please note that applications for Pre-k must include the following prior to being accepted:
 
  1. Completed Application
  2. Proof of Residence (2) ( utility bills, ID w/ address, tax doc., lease, or notarized document provided below)
  3. Parent ID 
  4. Income (2 consecutive check stubs for parents listed, statement of no or irregular income listed below)
  5. Child's Documents (Birth Certificate, Social Security Card, & Immunization Record
